Community Health Education
Knowledge is the first step toward health equity.
We meet people where they are — in their neighborhoods, their churches, their community centers — with health information that is clear, culturally relevant, and actionable. Through workshops, health fairs, and trusted community messengers, we translate complex health information into knowledge that empowers individuals.

Care Navigation
No one should navigate the healthcare system alone.
Our community health navigators sit down with individuals and families — in person, by phone, or in community settings — and help them understand their options, find local providers and programs, and take the next step with confidence.

- Follow-up and sustained community presence

Community voices deserve to be heard.
Advocacy at DaisyMe starts with listening. We hear what community members experience — the gaps in care, the barriers to access, the moments where the system falls short — and we take those experiences seriously. We share what we learn with the organizations and policymakers working to improve things.
