You have the right to know if information in a consumer report has been used against you. You have the right to know what is in your file. You have the right to ask for a credit score. You have the right to dispute incomplete or inaccurate information. Consumer reporting agencies must correct or delete inaccurate, incomplete, or unverifiable information. I hereby authorize DaisyMe Foundation and its designated agents to obtain a consumer report and/or investigative consumer report for volunteer screening purposes. This may include criminal history, sex offender registry, and identity verification.

I authorize release of information *

I authorize any person, organization, educational institution, law enforcement agency, or government entity to release information about me to DaisyMe Foundation or its authorized agents for the purpose of this background check.

I understand my rights under the FCRA *
